I am holding one SB A/C bearing no.325110100025456 with hospital Road branch, Jamnagar. My customer Id is 009578936.
Yesterday i.e. on 27/05/2015 I send my form for “PRADHANMANTRI JIVAN JYOTI YOJANA” dully filled & signed with my nephew. It was because I am at Bengaluru on long vacation & can’t personally go to Jamnagar branch. Concerned employee at the said branch asked him to provide proof of birth date. Hence I arrange to send the same to my nephew through email. My nephew again visited same branch today morning with said form & birth date proof. This time concerned employee asked to provide customer ID which I conveyed phonetically to him, but with my surprise, that employee on duty denied to accept my form & asked to call me personally to submit the form. Same thing was said by branch manager on contacting him by my nephew. And I can’t submit my form with the bank.
Please let me know what should I do to submit my form with bank before 30/05/2015, the last date. I also request you to take appropriate actions as you may deem fit against your employee/branch manager of concerned branch for not providing required service to me & haranguing me as an customer.
